Whether you’re an experienced clinician or a new graduate beginning your PT career, you’ll join a people-first organization where collaboration, professional growth, clinical autonomy, and patient-centered care are part of the everyday experience.

At H2 Health @ Berea, you’ll work in a 2,500-square-foot outpatient clinic alongside an experienced therapy team with more than 80 years of combined experience. The clinic serves a diverse general outpatient population and offers programs across orthopedic and musculoskeletal rehabilitation, balance and dizziness, sports medicine, chronic pain, neurological conditions, manual therapy, Workers’ Compensation, and other rehabilitation needs.

Your Role

  • Evaluate patients and develop individualized, evidence-based Physical Therapy plans of care
  • Treat a diverse outpatient caseload focused on mobility, strength, function, and return to activity
  • Provide therapeutic exercise, neuromuscular re-education, manual interventions, and functional training
  • Educate patients on movement, recovery, injury prevention, and home exercise programs
  • Collaborate with clinicians and support staff to coordinate high-quality care
  • Track patient progress and adjust treatment plans to support meaningful outcomes
  • Complete timely and accurate clinical documentation
  • Doctor of Physical Therapy (DPT) degree or equivalent legacy degree (MSPT/MPT/BPT) from a CAPTE-accredited Physical Therapy program
  • Current Kentucky Physical Therapist license or eligibility for Kentucky licensure
  • CPR certification, if applicable
  • Strong communication and collaboration skills
  • Commitment to high-quality, patient-centered care
  • New graduates are welcome to apply; previous outpatient experience is not required
