Job DetailsJob Location: Augusta - Augusta, ME 04330Position Type: Full TimeSalary Range: $27.00 - $29.50 HourlyThe scope of practice for this position is to provide professional care coordination service to eligible clients. The Care Coordinator (CC) is an advocate for clients when securing services and is responsible for coordinating all services deemed necessary to meet the needs of clients served. The CC will be sensitive toward others and enjoy working with children and adolescents and work effectively within an interdisciplinary team. This position will be required to possess the ability and desire to work flexible hours to meet client demand.
