Licensed Physical Therapist (PT)

Full-Time | Senior Living Community in Altadena CA

We are seeking a compassionate and motivated Licensed Physical Therapist (PT) to join our

rehabilitation team in a thriving senior living community. This is an excellent opportunity for a PT

who enjoys building meaningful relationships with older adults and wants to work in a

supportive, collaborative environment with opportunities for professional growth and

advancement.

Our community serves residents across Independent Living, Assisted Living, and Memory Care,

allowing you to provide meaningful therapy while developing strong relationships with residents,

families, caregivers, and the interdisciplinary team.

What You’ll Do

  • Provide high-quality, individualized physical therapy evaluations and treatments for older

adults.

  • Work with residents across Independent Living, Assisted Living, and Memory Care.

  • Develop and implement evidence-based treatment plans based on each resident’s

functional goals.

  • Address mobility, balance, strength, endurance, gait, fall prevention, transfers, and

functional independence.

  • Collaborate with caregivers, nursing staff, physicians, and other rehabilitation

professionals.

  • Educate residents and caregivers on safety, mobility, and home exercise programs.

  • Complete required documentation accurately and in a timely manner.

  • Monitor progress and modify treatment plans as clinically appropriate.

  • Participate as a valued member of the senior living community and interdisciplinary care

team.

What We’re Looking For

  • Current California Physical Therapist (PT) license in good standing.

  • Experience working with older adults is preferred, but motivated PTs with other clinical

backgrounds are encouraged to apply.

  • Strong communication and interpersonal skills.

  • Compassionate, patient-centered approach to care.

  • Ability to work independently while collaborating effectively with a multidisciplinary team.

  • Reliable, professional, and organized.
