What You'll Do

• Connect patients to healthcare and community resources

Qualifications

• High school diploma or GED
• Strong communication and relationship-building skills
• Knowledge of community resources preferred
• Bilingual (English/Spanish) preferred
• CHW certification or willingness to obtain certification preferred
