Physical Therapist Opportunity:

• Full-time, permanent position
  
• Schedule: Full-Time, Day Shift
  
• Setting: Both Inpatient and Outpatient care

Physical Therapist Responsibilities:

• Complete patient assessments to develop and implement an individualized care plan
  
• Provide high-quality, patient-centered care in collaboration with the patient, family, and care team
  
• Provide skilled, goal-oriented physical therapy while maintaining an ongoing assessment of the patient to make any changes to the care plan as necessary
  
• Maintain all records and reports per regulatory guidelines and coordinate with interdisciplinary teams
  

Physical Therapist Qualifications:

• BLS Certification
  
• Licensed Physical Therapist in the state of Ohio (or eligibility to obtain upon hire)
  
• Must have previous experience with Orthopedics, Neuro, and outpatient therapies
